Life & Disability
Apex pays for all of this in full. Your cost is nothing — but there are details worth knowing before you ever need to claim.

The details that affect a claim
STD hands over to LTD without a gap
The two are designed to connect. Short-Term Disability has a 7-day waiting period and then pays for up to 12 weeks. Long-Term Disability begins after a 90-day elimination period.
- Days 1–7
- Waiting period
- Day 8 to about day 91
- Short-Term Disability pays
- From day 91
- Long-Term Disability takes over
7 days plus 12 weeks lands at roughly day 91, which is where LTD picks up. You should not fall between the two.
Limitations on the LTD benefit
- Pre-existing conditions
- 3/12 limitation
- Mental health and substance abuse
- 24 months lifetime
A 3/12 pre-existing limitation generally means a condition treated in the three months before coverage began may not be covered in the first twelve months.
Two features of Basic Life people often do not know they have
Accelerated death benefit — if you are diagnosed as terminally ill you may be able to access 75% of your benefit, up to $250,000, while living.
Conversion — you may be able to convert your coverage to an individual policy when group coverage ends. This is time-limited, so ask before you leave rather than after.
Benefit reduction at age 70 — the Basic Life benefit reduces to 50% at age 70.
Check your beneficiary
Life insurance pays whoever is named on the policy, regardless of what a will says. After a marriage, divorce, birth or adoption, this is the single most commonly forgotten update — and the consequence falls entirely on the people you leave behind.
Beneficiaries are managed in Paylocity. It takes two minutes.
Two things that come with these plans

This fills a real gap in your medical coverage
Blue Cross Blue Shield Global Core gives you access to providers worldwide, but its own materials state there is no benefit for medical evacuation or repatriation — the single most expensive thing that can go wrong abroad.
AXA Emergency Travel Assistance covers exactly that. If you travel internationally, save the number before you go.
Employee Assistance Program through Equitable
Equitable includes an EAP at no additional cost, giving you short-term counseling and support. It is available to employees enrolled in Equitable company-paid or Equitable voluntary benefits.
Apex also provides the HealthJoy EAP separately, which is available to every employee whether or not you are enrolled in any benefits. The two cover different groups deliberately — if you are not on an Equitable plan, HealthJoy is your route.

Supplemental Life insurance
On top of the employer-paid Basic Life, you can buy additional coverage for yourself, your spouse or partner, and your children. You pay for this yourself.

Buy it while it is guaranteed issue
Up to $100,000 for yourself, $5,000 for a spouse and the full $10,000 for children requires no health questions. Above those amounts, evidence of insurability may be required — and that depends on your health at the time you apply, not now.
Enrollment windows are when you first become eligible, during any annual enrollment period, or within 31 days of a family status change.
Portability is the part people miss
Unlike Basic Life, Supplemental Life is portable — you can take it with you when you leave, as well as convert it to an individual policy. That matters if you ever change jobs while your health has changed.
Both options are time-limited after coverage ends, so ask before you leave rather than after.
Your cost
Supplemental Life rates are age-banded and depend on the amount you elect. Your actual per-paycheck cost is shown in Paylocity during enrollment.
