Vision
A $0 exam, a $150 frame allowance, and two benefits most people never claim.
Vision is the one benefit you pay for in full
Apex does not contribute to the vision premium — the employee pays 100%. The amounts below are the whole cost, not a share of it.
| Coverage tier | Your cost per paycheck |
|---|---|
| Employee Only | $3.91 |
| Employee + Spouse | $6.66 |
| Employee + Child(ren) | $6.85 |
| Employee + Family | $10.77 |
Vision premiums did not change at renewal.

Two things most people never claim
Diabetes eye care, paid in full
If you have been diagnosed with type 1 or type 2 diabetes, the plan covers up to two diabetic eye exams and diagnostic testing every 12 months, paid in full.
This sits on top of your routine exam. Diabetic retinopathy is the leading cause of preventable blindness in working-age adults, and it is asymptomatic early on — which is exactly why this benefit exists.
Children under 19
Kids get an exam at $0 copay, up to two per benefit frequency, and standard plastic lenses up to two per frequency.
- Standard polycarbonate lenses
- Paid in full
- Rx blue-light lens treatment
- Paid in full
Polycarbonate is the impact-resistant material normally charged as an upgrade. For children it costs nothing.
Additional in-network discounts
